Change of Name Application Instructions
This is the changing of name form.
Application Instructions:
- Applicant Signature: The applicant is to be the only one who signs with his or her old name. In the case of minors, parents/guardians are to sign, and there must be an Attested Legal Guardianship order, where possible.
- Self-Attestation: All certificates should be self attested.
- Eligibility: Applicant should be a native of this State and a declaration should be made.
- Reason to Change: There should be a logical cause of the change of the name.
- Fees: Rs. 750/- (G.O. Ms.No.250, dated 11.10.2023).
- Payment Method:
- In Person: e-Challan (no cash). Send counterfoil following payment.
- Post: Payments by e-Challan only (not by DD/Cheque/Postal Orders). Send the form with counterfoil.
- Working Hours:
- Forenoon: 10:00 a.m. to 1:00 p.m.
- Afternoon: 2:00 p.m. to 3:00 p.m. (no holidays)
- No Reminders: No reminders will be given.
- Application Form: Forms of this Department only will be accepted. No Xeroxes.
- Address: Check Door No. and PIN Code.
- Native District: Give district of birth.
- Punctuation: Punctuate initials and name correctly.
- Life Certificate: Life certificate should be attached by applicants 60+ years by a Government doctor.
- Alias Name: The new name cannot be an alias name.
- Evidence of Age: No birth certificate should be available, in that case, age certificate through a Government doctor.
- Photograph: Enclose a passport-size photograph with a self-attestation.
- Gazette: And go and collect Gazette within 5 days or it will be sent by post.
- Returned Gazettes: In case of return, retrieve in person within 6 months.
- In the event of Adoption: The adoptive father should sign and apply. Add adoption deed, birth certificate and ration card.
Enclosures:
- For Tamil Nadu Residents: Old name proof (Birth/School Certificate, Community Certificate) & residential proof (Ration Card, Voter ID, Aadhaar Card).
- Indian Residents: Birth certificate, ration card, voter ID, Aadhaar card, or residential certificate from Tahsildar.
- Change of Name Due to Adoption: Signed by the adoptive father/mother, with adoption deed, birth certificate, and ration card.
- Change of Name Due to Divorce: Attach certified judgment copy and marriage certificate (if remarried).
This proforma is intended for the change of name only.
If the change of name is due to a change of religion, a separate form (PROFORMA-II) is available and will be sent on application.
Conditions and Guidelines:
- Applicant Signature: The applicant alone should sign the application using their old name. In the case of a minor (under 18 years), the parents/guardian should sign in the provided space. For a guardian, an attested copy of the Legal Guardianship order must be enclosed. The relationship of the person signing the form must be mentioned in capital letters.
- Self-Attestation: All documents submitted must be self-attested by the applicant.
- Eligibility for Application: Notice for change of name will only be entertained for persons who are natives of this State. A declaration should be provided in the designated column.
- Reason for Name Change: A reason for the change of name must be provided in the application.
- Fees: The fee for the application is Rs. 750/- as per G.O. Ms.No.250, Tamil Development and Information (S&P2-1) Department, dated 11.10.2023.
- Method of Payment:
- In Person: Publication charges are accepted only in the form of an e-Challan (not in cash). Applicants must submit the counterfoil of the e-Challan after remitting the publication charges either online or offline at www.karuvoolam.tn.gov.in into the Government account.
- By Post: From 01-07-2023 onwards, publication charges will be accepted only through e-Challan. No Demand Draft, Postal Order, Cheque, or Money Order will be accepted. The counterfoil of the e-Challan must be submitted with the application form.
- Working Hours for Payment:
- Forenoon: 10:00 a.m. to 1:00 p.m.
- Afternoon: 2:00 p.m. to 3:00 p.m. (except Government Holidays)
- No Reminders: No reminders will be sent for remittance or for any other particulars.
- Application Form: Only the application form issued by this Department will be accepted. Xerox copies will not be accepted. Forms can be downloaded from the website.
- No Correspondence: No correspondence will be entertained with anyone other than the applicant.
- Address: Ensure the correct Door No. and PIN Code Number is written in the address section.
- Specify Native District: You must specify your native district (district of birth) in the application.
- Punctuation: Ensure that the punctuations between initials and names are correctly specified.
- Applicant's Signature: The applicant should sign using their old name only.
- Life Certificate: A life certificate may be obtained from a Government Medical Practitioner and must be enclosed in original if the applicant is 60 years or above.

- Alias Name: An alias name cannot be published as the new name.
- Rectification of Mistakes: Any discrepancies found in the Gazette can be rectified within six months. After that period, requests for corrections will not be entertained.
- Proof of Age: If the applicant does not have a birth or educational certificate, a certificate issued by a Government doctor as proof of age must be enclosed.
- Photograph: A recent passport-size photograph of the applicant must be affixed in the space provided, and it must be self-attested.
- Collection of Gazette: Applicants who wish to collect the Gazette in person must do so within 5 days from the publication date. If not collected, it will be sent by post.
- Returned Gazettes: If a Gazette is returned by the postal department, it will not be sent again. The concerned person can collect the returned Gazette from the office within 6 months from the date of publication.
- In Case of Adoption: If the change of name is due to adoption, only the adoptive father should apply and sign in the space provided for the applicant's signature.
Enclosures:
- For Andhra Pradesh Residents: A copy of the old name proof (Birth Certificate, School Certificate, Community Certificate) and residential proof (Ration Card, Voter Identity, Passport, Aadhaar Card).
- For Non-Andhra Pradesh Residents: Submit a certified copy of the birth certificate, ration card, voter identity, Aadhaar card, or a residential certificate issued by the Tahsildar.
- For Change of Name Due to Adoption: Signed by the adoptive father/mother, with adoption deed, birth certificate, and ration card.
- For Change of Name Due to Divorce: Attach certified judgment copy and marriage certificate (if remarried).
