A passport comprises several key particulars, including your name and address. So, if you have recently moved but your passport designates the old address, you must update it. Likewise, if your name is misspelled in the passport, it also needs an update at the earliest.
If somebody wants to substitute their middle name or last name, they must go to a Passport Seva Kendra, or the official website, apply for a different passport and take Form No. II for Assorted Services, and fill out the essential information.
Alongside the form, the aspirant will have to submit the following papers:
- Primary Passport along with a reproduction of the first two and last two pages that have been self-attested
- Founded on the motive for the alteration of name, the subsequent papers will have to be submitted:
- If a lady wants to name chnage after Marriage, she must present a reproduction of her mate's passport (if he has one), as well as a corroborated reproduction of the marriage license or other supporting certification, such as a joint photograph and an analogous affidavit (Annexure D').
- The aspirant for an annulment who wishes to swap their name must submit a legitimate copy of the divorce ruling together with an autographed affidavit or Deed Poll (Annexure E').
- A reproduction of the annulment decree or death certificate, along with the mark of the second marriage, must be submitted if the aspirant wants to alter his or her name following remarriage.
- If none of the above-mentioned circumstances apply, tender an affidavit or Endeavor Poll (Annexure E') together with an original advertisement for a name alteration in two daily newspapers (this is acknowledged only 30 days after printing of the said advertisement).
- The above must be presented either online or at the Passport Seva Kendra on the chosen day.
- The amended Passport will be sent to the claimant after a few days.

Cost for reissue of passport
- The application fee for a reissue of passport with 10 years validity is for 60 pages cost is ₹2000 and for 36 pages cost is ₹1500.
- For tatkal services individual has to pay ₹2000 additionally.
- For minors 36 pages cost is ₹1000 with 5 years validity and for tatkal ₹2000.
