"Where can I get the Aadhaar name change form?" is the question I hear every week or so.
The answer is: "there isn't one". The answer is, "There isn't". There is no PDF form that you download, fill and take to the office. The form is filled up at the office by the operator, or filled up online directly. You don't take a form, just documents.
This creates a lot of unnecessary footwork. This article will tell you what the Aadhar card name change form is, how it works in Karnataka, and how to avoid being ill-prepared for the process.
What Is the Aadhaar Name Change Form — And What It Isn't
For demographic changes, UIDAI has a single form: the Aadhaar Enrolment and Update Form. It's also known as Form 4. This latest version will help you update name, address, date of birth, gender, mobile number and email.

Now here's where most guides are a bit misleading: just downloading it and filling it out at home isn't enough. You'll go to a Seva Kendra and the operator will fill your details in another system (Aadhaar Update Client). They may refer to your form. And they will enter the data themselves.
So don't consider the form as an application. Think of it as an organisation tool to prepare for your appointment. Not as an application in itself.
If you want to update your data online, through myaadhaar.uidai.gov.in - you don't need to download the form at all. You type it in the browser.
When You Need the Physical Form vs. When You Don't
- Online via myAadhaar portal: No form The portal is the form. You fill the form with the new name, upload a document, and your form is submitted. This is good for small corrections when mobile is enrolled.
- At an Aadhaar Seva Kendra: The operator enters the form on their system. You can bring a printout of the Aadhaar Enrolment and Update Form to help you arrange your data - but the operator does not submit your form. They enter your details.
- At a bank branch with Aadhaar services: Same as Seva Kendra. The bank operator uses UIDAI client software. No need to fill the form.
- By post: Yes, but seldom used and has conditions. Covered separately below.

The current form is a single A4 page, double-sided. It covers all demographic fields. Download it, print it, and use it as a checklist before your Seva Kendra visit.

How to Fill the Aadhaar Name Change Form Correctly
- Write the name exactly as it appears in your supporting document. Not how you want it eventually. Not a combination of how it appears across two documents. Exactly as one document shows it. The operator will compare the form against your document character by character.
